Please remember to:

• Submit a completed application for each child you wish to register.
• List two (2) Emergency Contacts where asked. VA law requires two Emergency Contacts for each child registered for the program.
• Submit a completed General Health History to include:
• Immunization Record (found on the back of the General Health History form) for each child registered for the program, no matter the length of time.
• Physical Record for each child attending camp for more than 30 days (a copy of official school health form is acceptable).
• Provide Browne Academy/Summer Camp with Proof of Identity for each child registered for the program. (Form enclosed)
• Include the $50 processing fee with application.

Incomplete applications will not be processed.
